First line shows LossID, Date of Loss, TransferID and Status. Second line shows quantity lost, BookCode, Book, BookID and total cost.
Third line shows User/Teacher or StudentID, FirstName, LastName, Title, School, Street and Phone.
Fourth line describes the loss, fifth line describes the circumstances, sixth line is for comments and seventh line is
for resolution and approval. The footer counts the number of loss records. (rptLoss)
